Do you take health insurance?

I am currently in network with  United Healthcare, CareFirst/Blue Choice and Cigna. Because plans vary from policy to policy, it is important to call your insurance company to verify your benefits. You will be responsible for payment should your insurance decline to cover an appointment.  If you have an insurance that I am not participating in you can check to see if your policy has "out of network" benefits. If so, you will able to submit a claim for reimbursement. ​​I recommend contacting your insurance provider directly before scheduling an appointment to get a better idea of coverage and reimbursement costs. After each session I will provide you with a superbill that will have nutrition-related diagnosis code for you to submit to your insurance company for reimbursement.

What to Ask Your Insurance Company:

  • Do I have benefits to see a registered dietitian?

  • What is my co- pay ?

  • Is a preauthorization required ?

  • Do I have a deductible, and how much of the deductible have I met?

  • How many sessions are covered and within what time period?

  • What forms do I need to submit to qualify for reimbursement?

What forms of payment does Rebecca Thomas Nutrition LLC take?

Payment is to be made before services are rendered using either PayPal, Credit, Cash or Check. Checks are to be made out to: Rebecca Thomas Nutrition LLC
