Insurance and Payment Information

We are currently in-network with  United Healthcare, CareFirst/Blue Choice, and Cigna. 

Depending on which state you reside in, you may be able to use your insurance benefits.

 

We see clients in:AK, AZ, CA, CO,CT,HI,ID, IN, MA, MD,MI,NH,NY,OK,OR, PA, TX, VT, VA,WA,WV, WI,WY and accept Cigna, United Healthcare and Blue Cross/Carefirst insurance. We do not accept Medicaid or Medicare.

 

Because plans vary from policy to policy, it is important to call your insurance company to verify your benefits.

You will be responsible for payment should your insurance decline to cover an appointment.  If you have insurance that I am not participating in you can check to see if your policy has "out of network" benefits. If so, you will be able to submit a claim for reimbursement. ​​

 

we recommend contacting your insurance provider directly before scheduling an appointment to get a better idea of coverage and reimbursement costs.

 

After each session, you will provided with a superbill that will have a nutrition-related diagnosis code for you to submit to your insurance company for reimbursement.

What to Ask Your Insurance Company:

  • Do I have the benefits to see a registered dietitian?

  • What is my co-pay?

  • Is a preauthorization required?

  • Do I have a deductible, and how much of the deductible have I met?

  • How many sessions are covered and within what time period?

  • What forms do I need to submit to qualify for reimbursement?

Payment is to be made before services are rendered using either PayPal, Credit, Cash or Check. Checks are to be made out to: Rebecca Thomas Nutrition LLC
